What types of splints and orthotics are commonly used with CP? Resting hand splints, thumb spica splints, AFO’s, knee immobilizers, hip abduction wedges.
What is the purpose of treatment with orthosis and splints in CP? Maintain ROM, prevent contractures in specific joints, provide stability, and control involuntary movements.
What is the function of inhibitive casting? Immobilization, or to allow for weight bearing activity and tone reduction.
What is the function of serial casting? A conservative means of increasing joint ROM and functional movement.
What is the effect of serial casting on increasing ROM? Soft tissue is held in the lengthened state allowing for the addition of sarcomeres and gradually increased ROM.
What is the purpose of the three main types of surgical intervention for contracture management? To increase ROM through release, lengthening, or transfer of the affected muscles and tendons.
What is the postoperative treatment for soft tissue and bony surgical procedures? Immobilization for 6-8 weeks followed by splinting, therapy involves stretching and strengthening the affected muscles and strengthening the antagonist muscles.
What is the surgical intervention for scoliosis? Implanted rods or wires hold the spine in an improved alignment while grafted bone fuses it into the new position.
What is the surgical intervention for dislocated hips? A varus osteotomy involves altering the angle of the femur to place the head in the acetabulum which may need to be reshaped.
What is the primary goal of selective posterior (dorsal) rhizotomy? To decrease the spasticity of the legs in children with spastic diplegia.
How is selective posterior rhyzotomy (SPR) performed? L2-S1 rootlets are tested to determine the major contributors to the spasticity, these afferent sensory rootlets are severed leaving touch and position sense intact.
How is baclofen administered by a surgically implanted pump? A disc pump is under the skin of the abdomen and a catheter travels around to the back where it is inserted through the lumbar spine into the intrathecal space.
What are the advantages of the baclofen pump? Automatic dosing, reduction in spasticity, adjustable dosing.
What are the disadvantages of the baclofen pump? Mechanical failure, infection, need for close medical supervision.
What is the general purpose of adaptive equipment? Prevent abnormal postures while providing a stable foundation for the development of more skilled movement and better alignment.
What is the general purpose of augmentive and alternative communication devices? To provide purposeful communication and social interaction which are essential for the emotional health and development of children.
What is the purpose of nasogastric and gastrostomy tubes? To provide delivery of nourishment.
What is the difference between a nasogastric and gastrostomy tube? Nasogastric is for short term use.
